As of today the Wild poliovirus type 1 remains endemic in two countries: Afghanistan and Pakistan. In both, the obstacle is not vaccine availability but operational: armed conflict, mistrust of foreign-funded campaigns, and geographic remoteness in border regions.
There is also a second front. Variant poliovirus (cVDPV) — derived from the live attenuated oral vaccine in under-vaccinated populations — has emerged in parts of Africa, the Middle East, and (in small numbers) in countries that were previously polio-free. Stopping cVDPV requires the same playbook: surveillance, containment, mass immunization with the newer novel oral polio vaccine type 2 (nOPV2).
